AFFIRMATION OF EXTENSION OF THE REGISTRATION
OF A INDUSTRIAL DESIGN

In the matter of an application to extend the registration period
of a Registered Industrial Design

I, the undersigned, an officer who has been given the power to sign
and affirm on behalf of the Registrar, hereby certify that the following industrial
design which has been registered under the Industrial Design Act of the Year 1996
will continue to be valid with its registration period extended for 5 years as follows:
